Consent Forms

On admission and at the beginning of each academic year parents/carers are required to complete consent forms.

Please see the guide and links below to understand more about why and when the forms are required.

Copies of the form are available from the below links for you to print at home – copies may be printed at school and sent home for completion if requested.

Every form should be signed and dated by the person(s) with parental responsibility.

Data Sharing Consent Form

For parents to give the school consent to share data where it is deemed necessary

Annual Medical Consent

To provide the school with medical information for the year to ensure that pupils’ medical needs will be taken into account.

Administration of Medication Consent Form

To give permission to the school to administer short and/or long term medication to children.

If the child has a short or long-term condition requiring medication

For parents to request permission to take children out of school during term time. Parents are asked to avoid term time holidays.
